Questionnaire is the Tesi Group's fully WEB-based module for the creation and management of questionnaires.
The questionnaire can be used from any browser and from any device, including mobile devices; its pages are self-adaptive according to the type of screen.
The module allows the configuration of an indefinite number of questionnaires, each uniquely identified.
Questionnaire for each questionnaire allows the creation of questions and allows you to:
- Show the questionnaire information to the patient
- Group the questions into homogeneous groups according to question type
- Define the answer type for each question

• Yes/No
• Free text
• Single selection
• Multiple selection
• Date
• Number

- Define the compulsoriness of the question within the questionnaire
- Define conditions under which other questions can be made compulsory depending on the answer
- Configuration of the question according to the master data of the patient/donor (e.g. according to gender some questions can be displayed and others not)
- Define a hint that can help the patient/donor to understand the question

The patient/donor will receive by email the link to connect to the questionnaire and will be able to start answering the questions and will be shown the progress status and will be able to print the questionnaire.

INTEGRATION MODE
Questionnaire can be called up by other third-party software via a context call.
